Microscopic Ear Suction क्या है?

Microscopic Ear Suction एक procedure है जिसमें operating microscope (10-20x magnification) के under, ENT specialist fine suction tip use करके ear canal से wax, discharge, debris, या foreign body को carefully remove करते हैं। यह एक DRY method है – कोई water/liquid use नहीं होता।

कब Microscopic Ear Suction की ज़रूरत है?

1. Ear Wax Impaction (कान की मैल):

• Symptoms: Hearing loss, ear fullness (कान भरा हुआ), earache, tinnitus (सीटी)
• Types of wax:
  • Soft wax: आसानी से निकल जाता है
  • Hard/Impacted wax: सूख गया है, stuck – microscopic suction best
  • Complete blockage: Entire canal blocked – significant hearing loss
• Wax removal से immediately hearing improve हो जाती है!

2. CSOM – कान बहना (Chronic Ear Discharge):

• यह MOST IMPORTANT indication है!
• CSOM = Chronic Suppurative Otitis Media
• Symptoms:
  • कान से discharge (pus/mavaad) – foul-smelling
  • Hearing loss
  • Eardrum perforation
  • Recurrent infections
• Why microscopic suction essential:
  • Aural toilet: Pus, mucus, debris clean करना absolutely necessary
  • Medication effectiveness: Antibiotic drops तभी काम करेंगी जब ear clean हो
  • Healing promotion: Clean ear = better healing environment
  • Assessment: Perforation size, middle ear condition visualize कर सकते हैं

Microscopic Ear Suction Procedure – कैसे होता है?

Preparation:

• कोई special preparation नहीं
• अगर wax बहुत hard है: 2-3 days पहले wax-softening drops (ceruminolytic) दिए जा सकते हैं
• No fasting required

During Procedure:

Step 1: Patient Position:

• Examination chair पर comfortable बैठना
• Head को affected ear के side में tilt
• Relaxed position

Step 2: Microscope Setup:

• Operating microscope position किया जाता है
• 10-20x magnification setting
• Bright focused illumination

Step 3: Ear Canal Examination:

• Magnified view में ear canal examine
• Assessment:

Step 4: Suction Process:

Duration:

Is It Painful?

• Generally NOT painful
• Sensations:
  • Tickling sensation (common)
  • Slight pressure feeling
  • Suction sound (loud to patient but normal)
• Mild discomfort if:
  • Ear canal skin inflamed/infected
  • Foreign body impacted
  • Very hard wax removal

CSOM Management – Regular Suction की Importance

कान बहना (CSOM) में microscopic ear suction सिर्फ cleaning नहीं – यह active treatment का integral part है!

Complete CSOM Treatment Protocol:

1. Regular Aural Toilet (Ear Cleaning):

• Microscopic suction = Gold standard
• Frequency:
  • Active discharge: Weekly या twice-weekly
  • Improving: Every 2 weeks
  • Maintenance: Monthly
• What’s removed:
  • Purulent discharge (pus)
  • Mucus, debris
  • Unhealthy granulation tissue (fleshy growth)
  • Polyps (if small)

2. Topical Medications:

• Antibiotic ear drops (ciprofloxacin + steroid usually)
• ONLY effective if ear clean! – इसलिए suction जरूरी है

3. Systemic Antibiotics:

• BUT: Surgery से पहले ear को dry करना essential – regular suction से!

Why Regular Follow-up Essential:

• Single suction session से discharge permanently बंद नहीं होता
• Needs sustained effort: Regular cleaning + medications
• Timeline: कई weeks से months लग सकते हैं ear dry होने में
• Patient compliance important: Follow-up visits miss न करें!
• Gradual improvement: Discharge कम → ear dry → perforation stable/healing

After Ear Suction – क्या Expect करें

Immediately After:

• Hearing improvement! (if wax was blocking)
• Ear feels lighter, less full
• Slight wetness if drops applied
• Resume normal activities immediately

Post-Procedure Instructions:

• Keep ear dry: No water for 24-48 hours
• Medication compliance: Ear drops as prescribed (usually 2-3 times daily)
