Hearing loss can involve the outer or middle ear, the inner ear or hearing nerve, or more than one part of the hearing pathway. The appropriate next step depends on whether the change was sudden or gradual, one-sided or bilateral, and whether pain, discharge, tinnitus, vertigo or neurological symptoms are present.
Sudden hearing loss is urgent
A sudden new reduction in hearing—especially in one ear—needs urgent medical assessment. Seek emergency assessment for hearing change with new weakness, facial droop, speech difficulty, severe imbalance, fainting, major injury or another neurological warning sign.
Common reasons for hearing difficulty
- Earwax, ear-canal swelling or infection
- Middle-ear fluid, eardrum perforation or ossicular problems
- Age- or noise-related inner-ear hearing loss
- Sudden sensorineural hearing loss
- Medicine, illness, genetic or other less common causes
Assessment
A hearing assessment may include symptom history, ear examination and audiometry. Tympanometry or another age- and problem-appropriate test may be considered when it answers a specific clinical question. Confirm current test availability before travelling; not every patient needs every test.
Bring previous hearing tests, current medicines, hearing devices and a note of when the change began. Tell the clinician about noise exposure, ear surgery, discharge, tinnitus, vertigo and whether the hearing change affects one or both ears.
Treatment depends on the cause
Some conductive causes may improve with treatment of wax, infection, fluid or a selected structural problem. Many sensorineural losses are managed rather than reversed. Depending on the diagnosis, the discussion may include medical treatment, hearing protection, hearing-aid assessment, communication strategies, surgery or referral for another implantable option. No treatment or device can guarantee an individual result.
Hearing aids and follow-up
A hearing aid should be selected and programmed from the hearing assessment, communication needs, dexterity, comfort and follow-up plan—not from an online product claim. Adjustment and fine-tuning can take time. Children and adults with complex or asymmetric hearing loss may need a broader pathway.
