  • Swallowing mechanism ठीक से काम कर रहा है?

3. Globus Pharyngeus (गले में गांठ/कुछ अटका हुआ महसूस होना):

• “गले में कुछ फंसा हुआ है” – बहुत common complaint
• Usually कुछ नहीं होता (anxiety, acid reflux)
• लेकिन VLS important है rule out करने के लिए कि कोई actual growth तो नहीं

4. Breathing Problems:

• Stridor: Noisy breathing (सांस लेते समय आवाज़)
• Airway obstruction
• VLS से देखते हैं:
  • Vocal cords normal position में हैं?
  • Airway में कोई blockage (tumor, polyp)?
  • Paralysis (dono cords बंद position में अटके हुए)?

5. Chronic Cough/Throat Clearing:

• लगातार खांसी (balgam नहीं)
• हमेशा गला साफ करना पड़ता है (khich-khich)
• VLS checks: Vocal cord irritation, nodules, reflux damage

6. Screening for Cancer:

• High-risk patients:
  • Smokers (especially >20 years smoking)
  • Alcohol users
  • Both smoking + drinking (highest risk!)
  • Age >50 years
• Even if no symptoms – VLS recommended for screening
• Early detection saves lives!

7. Voice Professionals:

Types of Videolaryngoscopy – दो Methods

Method:

What to Expect:

• नाक में local anesthetic spray (सुन्न करने वाली दवा) लगाई जाती है
• Taste bitter हो सकता है

• नाक में थोड़ा pressure/discomfort feel हो सकता है – pain नहीं

2. Rigid Laryngoscopy (मुंह के रास्ते):

Method:

• एक rigid (कठोर) metal telescope मुंह में रखा जाता है
• Tongue पर rest करता है

Advantages:

• Superior image quality: Flexible से ज्यादा bright, high-resolution, magnified image
• Subtle details (छोटे changes) better visible
• Lesions की exact nature देख सकते हैं

Limitations:

• Normally बात नहीं कर सकते (usually only “eeee” sound कर सकते हैं)
• Gag reflex (उल्टी आने का sensation) trigger हो सकता है
• कुछ patients को uncomfortable लगता है

When Used:

• जब बहुत detailed image चाहिए
• Lesion की exact characteristics देखनी हों
• Usually flexible के साथ combination में

Special Feature: Stroboscopy – “Slow Motion” में Vocal Cords

What is Stroboscopy?

Videolaryngoscopy के साथ अक्सर Stroboscopy technology भी use होती है। यह एक amazing feature है!

The Problem:

• Vocal cords vibrate बहुत तेज़ – 100-1000 times per second!
• इतनी fast vibration human eye directly देख नहीं सकती
• Normal VLS में: Cords blurred दिखती हैं

The Solution – Stroboscopy:

• Flashing light use करता है

VLS Examination – क्या होता है? (Step by Step)

Before the Procedure:

Preparation:

• No major preparation needed!
• कुछ खास fasting नहीं
• Normal खाना-पीना OK (procedure से कुछ घंटे पहले तक)
• Comfortable clothes पहनें

What to Bring:

• Previous medical records (if any)
• List of current medications
• कोई साथ आए (driving के लिए – throat numb होती है)

During the Examination:

Step 1: Positioning

• आप chair पर बैठेंगे
• Slightly forward lean
• Comfortable position

Step 2: Local Anesthesia

• Doctor नाक या गले में anesthetic spray लगाएंगे (lignocaine usually)
• Purpose: Area numb (सुन्न) हो जाए, discomfort कम
• Taste bitter हो सकता है
• गला thick/swollen महसूस होगा – यह normal है (numbness का sensation)
• 2-3 minutes में effect आ जाता है

Step 3: Scope Insertion

For Flexible Laryngoscopy:

• Scope नाक से gently pass होता है
• नाक में pressure महसूस होगा
• Deep breath through nose लें – helps
• Few seconds में scope position में

For Rigid Laryngoscopy:

• Doctor आपकी tongue को gauze से gently hold करेंगे
• Telescope मुंह में place होगा
• आपको “eeee” sound करने को कहा जाएगा (epiglottis उठता है, better view)

Step 4: Examination

• Doctor screen पर देख रहे हैं
• आपको different sounds करने को कहा जाएगा:
  • “Eee” (high pitch)
  • “Ahh” (opens throat)
  • Counting (1, 2, 3…)
  • Singing (singers के लिए)
  • Swallowing (निगलना)
• यह सब vocal cord movement assess करने के लिए
• Images/video record हो रही है

Duration:

• Total time: 5-10 minutes usually
• Actual scope insertion: 2-3 minutes
• Very quick!

Is It Painful?

Generally NOT painful, but can be uncomfortable:

• Flexible scope: Most patients tolerate easily
  • नाक में pressure sensation
  • Minor discomfort
  • Pain नहीं (anesthesia के कारण)
• Rigid scope: More uncomfortable
  • Gag reflex trigger हो सकता है
  • लेकिन anesthetic spray helps prevent this
  • थोड़ा patience चाहिए
• Children: Usually cooperative हो जाते हैं gentle handling से

After the Procedure:

Immediate After:

• Scope निकाल दिया जाता है
• कुछ minutes rest
• गला numb रहेगा (30-60 minutes)

⚠️ Important – Eating/Drinking:

• Do NOT eat या drink जब तक sensation वापस न आ जाए
• Usually 30-60 minutes लगते हैं numbness जाने में
• Why: Numb throat = aspiration risk (खाना windpipe में जा सकता है)
• Test करें: Sip थोड़ा water, अगर normally निगल सकते हैं = OK

Results & Discussion:

• IMMEDIATE! यही VLS की beauty है

• Explain करेंगे findings:
  • Vocal cords normal हैं या कोई problem?
  • Redness, swelling, nodules, polyps, mass?

VLS vs Microlaryngoscopy – Difference समझें

Common Conditions Diagnosed by VLS

1. Vocal Cord Nodules (“Singer’s Nodules”):

• Benign (non-cancerous) callus-like growths
• Both vocal cords पर (bilateral), symmetric
• Cause: Voice abuse (shouting, singing with improper technique)
• VLS appearance: Small white/pink bumps on cord edge
• Treatment: Voice therapy usually; surgery rarely needed

2. Vocal Cord Polyps:

• Softer than nodules
• Usually one cord (unilateral)
