PTA Test क्यों करवाना चाहिए?

Common Symptoms – जब PTA जरूरी है:

1. “कम सुनाई देता है” (Hearing Loss):

• धीरे बोलने वाले लोगों की बात समझ नहीं आती
• “ज़ोर से बोलो” बार-बार कहना पड़ता है
• Family complains “आप सुनते नहीं हो”

2. TV/Phone Issues:

• TV volume बहुत तेज़ रखते हैं (family परेशान)
• फोन पर बातचीत मुश्किल
• एक कान से phone better सुनाई देता है

3. Noisy Environments:

• भीड़ में, functions में बातचीत नहीं समझ आती
• “Background noise में problem होती है”

4. One-Sided Hearing Loss:

• एक कान से कम सुनाई देता है
• Unilateral hearing loss (sudden या gradual)

5. After Ear Infection/Surgery:

• Chronic ear infection (CSOM) के बाद
• Ear surgery के बाद hearing check

6. Tinnitus (कान में आवाज़):

• कान में सीटी/भनभनाहट के साथ hearing loss

7. Children:

• बच्चा respond नहीं करता normally
• Speech delay (बोलने में देरी)
• School performance issues

8. Elderly:

• Age-related hearing loss (Presbycusis) common >60 years
• Gradual progressive hearing loss

9. Occupational:

• Noisy workplace (factories, construction)
• Regular screening recommended

10. Baseline:

• Starting ototoxic medications (कान को harm कर सकती हैं)
• Before chemotherapy
• Future comparison के लिए baseline audiogram

PTA Test कैसे होता है? – Step by Step

Preparation:

• कोई special preparation नहीं चाहिए!
• Normal खाना-पीना OK
• कान साफ होना चाहिए (wax नहीं होना चाहिए – doctor check करेंगे)
• अगर cold/ear infection active है → inform doctor (affect कर सकता है result)

Test Environment – Soundproof Booth:

• PTA को soundproof booth में किया जाता है
• Why: बाहर की noise test results को affect नहीं करनी चाहिए
• आप booth के अंदर बैठेंगे
• Audiologist बाहर से control करते हैं
• Glass window से देख सकते हैं आपको

Test Procedure:

Step 1: Otoscopy (Ear Examination):

• Doctor पहले ear canal check करते हैं
• Wax impaction है तो remove करते हैं
• Eardrum देखते हैं
• Clean ear = Accurate test

Step 2: Headphones/Earphones:

• Special audiometry headphones पहनाए जाते हैं
• या insert earphones (कान में fit होने वाले)
• Comfortable position

Step 3: Instructions:

• Audiologist समझाते हैं: “जब भी कोई आवाज़ सुनाई दे (चाहे बहुत हल्की), तुरंत हाथ उठाएं या button press करें”
• आवाज़ बहुत soft होगी, ध्यान से सुनना होगा

Step 4: Testing Different Frequencies:

• Audiologist different pitches (frequencies) की beep sounds play करते हैं
• Frequencies tested: Usually 250 Hz, 500 Hz, 1000 Hz, 2000 Hz, 4000 Hz, 8000 Hz
• Each frequency पर volume धीरे-धीरे कम करते जाते हैं
• जब तक आप सुन सकें उस softest sound तक
• आप हाथ उठाते रहें जब सुनाई दे

Step 5: Both Ears Separately:

• पहले one ear test, फिर दूसरा
• Each ear की separate audiogram बनती है

Step 6: Air Conduction vs Bone Conduction:

A. Air Conduction (पहले):

• Headphones से – normal hearing pathway
• Sound → Ear canal → Eardrum → Middle ear → Inner ear

B. Bone Conduction (अगर Air conduction abnormal है):

• एक vibrator आपके mastoid bone (कान के पीछे की हड्डी) पर रखा जाता है
• Vibrations directly inner ear को stimulate करती हैं
• Bypasses outer and middle ear
• Purpose: Differentiate between:
  • Conductive loss (outer/middle ear problem)
  • Sensorineural loss (inner ear/nerve problem)

Test Duration:

• 15-20 minutes typically
• Cooperative adults: 15 min
• Children: थोड़ा ज्यादा time (patience चाहिए)

Is It Painful?

• बिल्कुल नहीं! Completely painless
• Non-invasive
• बस ध्यान से सुनना होता है

Understanding Audiogram – Results कैसे Interpret करें

What is an Audiogram?

• एक graph जिसमें:
  • X-axis (horizontal): Frequency (Hz) – 250 to 8000 Hz
  • Y-axis (vertical): Intensity (dB HL) – 0 to 120 dB
• Right ear = Red (O symbol usually)
• Left ear = Blue (X symbol usually)

Normal Hearing:

• 0-20 dB HL = Normal
• Graph line इस zone में = कोई hearing loss नहीं

Degrees of Hearing Loss:

• Mild (26-40 dB): Soft sounds miss होती हैं, whispering नहीं सुनाई देती
• Moderate (41-55 dB): Normal conversation मुश्किल, louder speech चाहिए
• Moderately Severe (56-70 dB): Loud speech भी मुश्किल, hearing aid beneficial
• Severe (71-90 dB): Very loud sounds only, hearing aid essential
• Profound (>90 dB): बहुत कम सुनाई देता है, powerful hearing aid या cochlear implant

Types of Hearing Loss (Audiogram से पता चलता है):

1. Conductive Hearing Loss:

• Problem: Outer या Middle ear में
• Audiogram pattern:
  • Air conduction (headphone) = Abnormal (loss दिखता है)
  • Bone conduction (vibrator) = Normal
  • Air-Bone Gap present
• Common causes:
  • Ear wax impaction
  • Eardrum perforation
  • Middle ear fluid (OME)
  • Chronic ear infection (CSOM)
  • Otosclerosis
• Good news: Usually surgically correctable!

2. Sensorineural Hearing Loss (SNHL):

• Problem: Inner ear (cochlea) या auditory nerve
• Audiogram pattern:
  • Both air और bone conduction = Abnormal (same level)
  • No Air-Bone Gap
• Common causes:
  • Age-related (Presbycusis) – most common
  • Noise-induced
  • Genetic
  • Infections (meningitis)
  • Ototoxic medications
• Treatment: Usually permanent, hearing aids help

3. Mixed Hearing Loss:

• Both conductive + sensorineural components
• Air-bone gap + bone conduction also abnormal

Configuration (Pattern):

• Flat: सभी frequencies equally affected
• Sloping: High frequencies ज्यादा affected (age-related common)
• Cookie-bite: Mid frequencies affected
• Noise-notch: 4000 Hz पर dip (noise-induced characteristic)

After PTA Test – क्या होता है?

Immediate Results:

• Audiogram तुरंत available
• Doctor/Audiologist explain करेंगे

ENT Consultation:

• Slightly different approach – game-based

• Parent cooperation essential

Elderly:

• May need more time
• Instructions clear और loud
• Sometimes cognitive issues make test challenging
